- For donating items to the SJLES PTA
- You can submit this Donation Form directly with your donation or via email to PTA VP of Finance and Compliance at [email protected]
- Event Summary and Deposit Form
- For committee Chairs to fill out after each event
- Fill out each applicable section of the form (Expenses, In-Kind Donations, Revenues) and deliver it and any funds for deposit to the current treasurer found on Board of Directors. The treasurer can be contacted at [email protected]
- PTA Debit Card Purchase Request Form
- To be used by volunteers or school staff who wish to purchase services or other items using the company debit card.
- Form should be filled out and sent to [email protected] for approval.
- Completed form is sent to [email protected] for secondary authorization and payment processing.
